Does IDV use scale_factor and add_offset when it's imaging a 2-d netCDF field? In particular, does it use them with the latitude and longitude variables? It doesn't list these variables among the 2-d fields, so I presume something is happening with them by default, but none of the images I try to make are even vaguely sensible so it doesn't look as if they are getting used in the projection mapping process.
